Statutory declaration

The information in an application for a provisional or final certificate must be certified as true and correct in a proforma statutory declaration.

This must be sworn by a person from the applicant company who is in a position to attest to the veracity of the application form and its attachments – normally this would be a company director, often the physical producer – and witnessed by somebody authorised at law to do so.
